LVN - Care Coordination and Resource Services - Full Time - 8 Hour - Days

Job Description: Under the supervision of the Care Coordination and Resource Services Supervisor, the LVN is responsible for post discharge follow up calls to ensure discharge plan is followed. Assists with arranging appropriate follow up appointments as needed and reviews the after visit summary, including medication reconciliation. The LVN works in collaboration and in conjunction with the RN and Care Coordination and Resource Services Team to facilitate appropriate care and interventions. The LVN provides follow-up care as needed, as well as pertinent patient education, referral to relevant educational opportunities and information sources designed to maintain function, prevent illness, and promote wellness.

Education

• Graduate of accredited Licensed Vocational Nurse Program

Experience

• Two (2) years of clinical experience working in a medical setting with a diverse patient population - Required.

• One year of care coordination health plan or ambulatory - Independent Physician Practice (IPA) or Medical Group or case management experience - Preferred

Certifications/Licensures

• LVN Licensed Vocational Nurse - California Board of Vocational Nursing - Required

• BLS Basic Life Support - American Heart Association - Required

Work Shift: 08.0 - 08:00 - 16:30 No Waive (United States of America)

Pay Range:

$38.41 - $51.85 Hourly

Offer amounts are based on demonstrated/relevant experience and/or licensure. Pay will be adjusted to the local market if hired outside of the Bay Area .
Note: Positions at JMH which are exempt (not eligible for overtime) under the level of Manager are listed as hourly for compensation purposes on this posting. The work shift will contain the word 'exempt' on it.
