Details:
Setting: Provide Hospice/Palliative Care for Patients
Work autonomously in an APP role
Work collaboratively with the Palliative Care Medical Team to deliver appropriate care from initial assessment through discharge
Assessments include: disease-specific changes, decline or change in function, nutrition and/or cognition, inadequately-controlled pain or other distressing symptoms, falls, infections, impact of disease burden on patient, caregiver, and/or family's quality of life
Prescribe medications, order/interpret labs, order treatments or medical equipment
Have meetings with Palliative Medical Director as needed, but no less than biweekly
