The services include one program, called Tandem Care, that handles complex patients with diabetes or cardiovascular disease and a behavioral health condition. In this program the Psychiatrist and the Care Manager meet with each of the primary care providers each week for 30 minutes to discuss each of their patients who are up for review. The psychiatrist and the primary care provider work together to make recommendations on medications and the care manager follows up with the patient.Most of the work week would be in the psychiatry department.
