SUMMARY DESCRIPTION OF PROGRAM

Heartland Bridges houses two programs, Bridges Transitional Living and Bridges Crisis Care. Transitional Living is a short-term transitional housing program enriched with supportive services designed to assist people with a diagnosed mental health or co-occurring condition while they transition to stability in the community. Crisis Care is a residential stabilization service designed for individuals who need a safe, secure environment for 3-5 days while stabilizing from a crisis before returning to the community where they live.

SUMMARY DESCRIPTION OF WORK

The role of the Case Manager I - Overnight is to provide facility oversight and assist in duties to ensure the day-to-day operations of the program. Decision making, problem solving, and highly developed interpersonal skills are critical.

Compensation: between $16.31 and $19.44 per hour (wage is based on total years of relevant experience)
Work Schedule: Two overnight shifts (20 hours) per week, from 10:00 p.m. to 8:00 a.m.

MINIMUM QUALIFICATIONS

  • High School diploma or associate degree
  • Two years of experience in human services
  • Valid driver’s license and acceptable driving record

    ESSENTIAL DUTIES AND RESPONSIBILITIES

    • Develops supportive relationship with clients and responds to client needs during shift
    • Documents client activity per established record keeping procedures
    • Monitors client self-administration of medication and follows procedures of recording
    • Executes full medication count on nightly basis
    • Answers crisis care referral calls, assesses individual by gathering information and provides appropriate and accurate application verdict, maintaining strict confidentiality
    • Completes enrollment/intake on eligible clients
    • Conducts nightly housing lead searches
    • Psycho-educates clients on life, social and coping skills
    • Provides problem solving and crisis intervention services to empower clients to develop self-sufficiency
    • Utilizes trauma-informed care, person centered and culturally competent principles in providing services to clients
    • Demonstrates knowledge of current community and HFS programs and resources to meet client needs
    • Completes monthly client record reviews
    • Conducts client room searches per program protocol
    • Helps ensure room readiness for new clients
    • Orients clients to Bridges protocols and addresses client non-compliance as necessary to create a safe and secure environment for all residents and staff
    • Attends All Staff meetings as scheduled
    • Responsible for maintenance and upkeep of facility
    • Able to work both independently, with little supervision, and as a part of a team
    • Always displays a courteous and caring attitude to the clientele, volunteers, and visitors of the Agency
    • Abides by all specific program and Agency procedures, policies, and requirements
    • Cooperates and collaborates with program area staff, volunteers, and other Agency staff
    • Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media
    • Create, maintain, and share as appropriate a dynamic self-care plan
    • Essential functions of this job are to be performed on company physical work site
    • Performs other program related duties as assigned
