Our client is seeking a RN Case Manager to join their growing team. The selected individual will be responsible for assessing, planning, securing, and delivering health and habilitation support for people and will accept responsibility for the direction, monitoring, and delegation of activities related to the provision of medication and additional activities by medication aides. Essential Functions

  • Provide professional nursing care utilizing all elements of the nursing process while maintaining confidentiality.
  • Oversee and ensure the health, safety, and well-being of the people supported in their homes and/or day programs, thereby being attentive to personal choices and interests.
  • Perform the following functions related to the people on the case management caseload:
    • Assessment - Uses physical assessment skills and the data collected from all available resources to analyze and identify needs and appropriate actions.  Performs ongoing assessments and observations.  Identifies and documents changes in the person's health status that will affect the unlicensed employee's ability to meet needs. Review annually prior to annual ISP or when significant medical changes occur.
    • Planning - Develops individualized care plans and Collaborates with the person, employees, health care professionals and the ISP team to establish an individualized care plan with person-centered goals and outcomes to achieve optimal health.
    • Implementation – Delegates, accepts responsibility for delegating, and monitors the work of unlicensed employees and ensures that employees implement interventions based on the established individualized plan of care.  Collaborates with other health care professionals and communicates with unlicensed employees to assure that the person's needs are being met. 
    • Evaluation - Effectively evaluates all aspects of the individualized plan of care and makes changes as necessary. 
  • Teach employees appropriate special care procedures, training on medical equipment, care of the person, medication issues, retraining etc. as needed.
  • Audit medical documentation and medication records.
  • Communicate with physicians, vendors, social workers, and supervisors etc. as needed to provide quality care for the person.  This task may be delegated as appropriate.
  • Facilitate communication of information between hospital and employees during hospitalizations.
  • During work hours accept reassignment to provide direct support at various locations, as need for situations when unexpected staffing needs arise, emergency response is needed, or if additional staffing is required.  
  • Attend hospital discharge meetings, in-services, educational programs, and essential agency training.
  • Participate in required ISP meetings on nursing caseload and cover other nursing caseloads as necessary.
  • Assume medical on-call rotation responsibility evenings and weekends.
  • Follow HIPPA guidelines to ensure confidentiality.
  • Assist in the orientation and training of new employees. 
  • All other duties as assigned, which are necessary and appropriate for the provision of quality services.

Requirements

  • Graduate of an accredited School of Nursing and State of Nebraska licensed Registered Nurse or current RN licensure from a state participating in the multistate privilege to practice compact with Nebraska required.
  • Minimum two years’ experience as an RN with experience within the two years prior to date of application preferred.
  • Ability to pass a criminal background and Adult Protective/Child Protective Services check.
  • Must possess and maintain a current, valid driver’s license, pass a driving record check, and may, depending on agency needs, be required to have an automobile with required state insurance coverage available for purposes of transporting persons in services.
  • Ability to keep thorough and legible professional documentation of medical issues while ensuring timely and accurate completion of required documentation. 
  • Employ effective communication, both oral and written. 
  • Ability to support and relate comfortably, confidently, and effectively with people with intellectual disabilities.
  • Self-directed with the ability to work with minimal direct supervision yet respond to and seek supervision when needed.
